  Hazel Van Dyke, mother of actors Dick and Jerry Van Dyke, has died at the age of 95. Mrs. Van Dyke, who lived in Malvern, Arkansas, died Sunday at Baptist Medical Center in Little Rock. Since 1984, she had been living in Malvern with her son Jerry, a co- star on the ABC comedy series ''Coach.'' She was the widow of L.W. (Cookie) Van Dyke.
